Community Health Needs Assessment Reports

UnityPoint Health completes Community Health Needs Assessments (CHNA) for our hospital facilities every three years. This assessment identifies key issues and health needs affecting the people in the communities we serve.

The CHNA is a hospital’s foundation for planning and allocating resources for the community's health and acts as a baseline for assessing the progress of hospital and community health objectives. Hospitals then work with community partners to develop and implement a plan that addresses the health issues identified in the CHNA.
